Renewal Process
- Renew by clicking on the appropriate Level below, and you will receive a payment confirmation receipt via email. Click here to review the required documentation.
- Login using your email address and password. You should have already received two emails from VeriREP; one includes your password and the other includes a link to the online application.
- Submit your outstanding renewal documents via Transfer Files and use the Hospital Policy page to review, sign and submit Hospital Policies.
- Expirable documents include: PPD, Health Assessment (Level III only), and Certificate of Company general liability insurance.
- Once your renewal process is complete VeriREP will mail your ID badge to the address listed on your application.
*If you are already a registered supplier with VeriREP you do not need to pay another fee for servicing more than one facility. The fee is per year not per facility.
