Registration Process
- Register by clicking on the appropriate Level below, you will receive a payment confirmation receipt via email. Click here to review the required documentation.
- Following the payment confirmation email you will receive two additional emails; one includes your password and the other includes a link to the online application.
- Login using your email address and the password that was provided.
- Complete the initial application and submit all required documentation via Transfer Files and use the Hospital Policy page to review, sign and submit Hospital Policies.
- A VeriREP Specialist will review and verify your submission and you will be contacted once it is complete.
- When your process is complete VeriREP will mail you your ID badge.

*If you are already a registered supplier with VeriREP you do not need to pay another fee for servicing more than one facility. The fee is per year not per facility.
